When you go through the assessment you indicate a number of personal items such as weight, height, food preferences and so forth. PHCC LP does not share that data with your employer, insurer or any third party. If any information about your use of the program is needed by your employer or insurer so that they can pay for the program there will be a special privacy policy disclosing the full extent of what information will be shared and how your information will be used.

If you are uncomfortable with that privacy situation you can just pay for the program directly and not use an employer or insurer code, or not answer any questions during the assessment that you are not comfortable with.

Categories of Personally Identifiable Information We Collect

We collect personally identifiable information, as defined above, from you when you register on our website, or place an order with us. At some future point we may also collect personal information about you from other third parties, such as demographic firms, to make our future marketing efforts more efficient.

PHCC LP collects information in aggregate form from online visitors, including information about the total number of visitors, interests in various aspects of our products or services and activities that users engage in during sessions on this website. This information helps us maintain and improve our sites. Personally identifiable information such as your name is not used in this process of analyzing aggregate user data.
